Nobody had ever suggested to Marek Doubek that a prescription could get shorter. Six years of appointments had only ever added.

Evenings were the problem, as they usually are. Not the food itself so much as the eight-hour gap that preceded it.

Designing for month five

The fortnightly group ran with the same nine people throughout. Nobody was weighed in front of anybody. Most of the time was spent on what had gone badly.

Deprescribing takes longer than prescribing and gets a fraction of the clinic time. That’s a design fault, not a clinical one.

A year in

Maintenance is its own phase with its own skills, and the team treats it as harder than the losing phase rather than as a victory lap.

He is on two fewer agents than he was in January, and his readings are steadier than they were on all of them.
